Individuals enrolled in health insurance can change their registered initial examination and treatment location within the first 15 days of each quarter. The procedures are carried out through the Social Security Institution or the National Public Service Portal.
Health insurance participants have the right under the law to change their initial examination and treatment locations within the first 15 days of each quarter (specifically in the months of January, April, July, and October). This change, made according to residence, workplace, or education status, is processed based on the health insurance card database. In case of discrepancies between paper and electronic card information, electronic card data is taken into consideration.
For the change procedures, a health insurance participation and information correction declaration form (Form No. 02) and a citizenship ID card are required. The submission location of the documents varies depending on the participant group. Employees submit their documents to the Social Security Institution or their employers, family health insurance participants to the collection service institution, and retirees, children, and other groups to the local Social Security institution that issued the card.
Individual participants can apply online through the National Public Service Portal. For the online procedure, after logging into the system, the application declaration is selected, the transaction code numbered 612b is found, and the requested medical examination and treatment facility information in section (17) is filled out to complete the transaction with a confirmation code.
