According to a new study, examining the blood biomarker p-tau217 and the APOE genotype together plays a critical role in predicting the timing of Alzheimer's-related cognitive decline. Experts also emphasize the importance of blood sugar and metabolic factors in brain health.

Scope of the Study and Indicators

Data from seven major studies involving 8,582 individuals from diverse ethnic backgrounds were pooled together. The two primary indicators examined were blood p-tau217 levels and the APOE genotype.

p-tau217 is a blood marker associated with amyloid and tau pathology accumulating in the brain during the Alzheimer's process and can start rising before forgetfulness begins.

Genetic Risk Factor and Process

The APOE4 variant is among the strongest known genetic risk factors for late-onset Alzheimer's.

In individuals carrying at least one copy of APOE4, the window during which cognitive symptoms may appear following the rise in p-tau217 extends to an average of 3-4 years.

Risk Increase Rates and Figures

Every standard deviation increase in p-tau217 levels raises the risk of new cognitive impairment by 76 percent in APOE4 carriers.

While the same rate of increase provides a 26 percent increase in non-carriers, it shortens the timeframes in the process by 23 percent and 13 percent, respectively.

Changing Dimension in Clinical Approach

Published in The Lancet Neurology, the research shifts medicine from the question of whether a disease exists to the question of when it might become clinical.

Experts emphasize that these tests are not standalone prophecies, and that general health monitoring and proper timing are of great importance.

Metabolic Health and Blood Sugar

Assoc. Prof. Dr. Hasan Armağan Uysal stated that focusing solely on mental processes in the care of Alzheimer's patients is an incomplete approach.

Drawing attention to the critical role of blood sugar, blood pressure, sleep, and nutrition on brain health, the importance of comprehensive care was advocated.

Assoc. Prof. Dr. Uysal clarified the concept of Type 3 diabetes, stating that it is not an official diagnosis.

It was conveyed that the concept is used to draw attention to the relationship between insulin signaling and metabolic disorders in the brain and Alzheimer's.
