Ho Chi Minh City Residents to Declare Health Information on Digital Platform
As the practice of data collection via paper forms comes to an end, citizens will be able to update their health status online through the Digital Citizen application.
The Ho Chi Minh City Department of Health announced that starting September 15, the process of collecting data via paper forms has been suspended, and citizens will self-report their health status digitally using the Digital Citizen application.
Era of Paper Forms Comes to an End
The Ho Chi Minh City Department of Health announced that local authorities have stopped issuing new health information collection forms and are intensifying efforts for citizens to self-declare their health status through the Digital Citizen application.
After this date, the function of entering data from paper forms into the public health management platform will be locked, and all subsequent information will be transferred to the system via online declaration.
Deadline for Past Forms
Local authorities are strongly urged to review and finalize the data no later than September 20 for information collection forms issued and collected before September 15 that have not yet been updated.
Digital Transformation and Active Participation
According to the Department of Health, this step is not only a change in data entry methods but also a major transformation marking the shift from a manual model to a digital environment in people's health management.
Over the past six weeks, local authorities have collected information from citizens who underwent health checks and screenings in 2026 in accordance with instructions.
Self-Declaration Process via the Application
In coordination with the City's Digital Transformation Center, the Department of Health has launched a self-declaration function for health check and screening information on the Digital Citizen application.
Individuals who underwent a health check in 2026 but whose results have not been updated in their electronic health records can declare their information directly through the application.
Reduction of Healthcare Staff Workload
This method provides great convenience by eliminating the requirement for citizens to submit paper forms to local authorities.
At the same time, the workload related to the manual distribution, collection, and data entry of forms at health stations is significantly reduced.
Current Health Screening Data
The Department of Health stated that as of September 13, approximately 4.5 million people in the city had undergone health checks and screenings, corresponding to about 34.8% of the population.
As a result of these screenings, electronic health records have been successfully created for over 4.1 million people.
