A comprehensive public inquiry conducted in New Zealand revealed that authorities did not take the harm Tom Phillips inflicted on his children seriously enough and failed to protect them.
Delays in Intervention by Authorities
It was determined that relevant agencies, such as the police and Oranga Tamariki, ignored or downplayed the harm Phillips caused to the children and were slow to initiate search efforts.
It was found that significant concerns raised by the maternal family were minimized by the authorized institutions and that the warnings from the mother's side were largely ignored.
Failure to Evaluate Threats and Tips
Despite warnings regarding his arming and anti-government and anti-vaccine views, it was emphasized that Phillips's state of mind and beliefs were not taken seriously enough.
The failure of the police to conduct a rapid follow-up after a public tip reported near Marokopa in May 2023 resulted in a major missed opportunity.
Institutional Apology and Acceptance of Recommendations
Relevant authorities and ministers accepted all nine recommendations and forty-one findings contained in the inquiry report and issued a public apology.
